How to Report an Incident or Near Miss (Office Employees Only)
If you are involved in or witness and incident or near miss (near miss is an event or hazard that does not result in injury, illness, or damage, but had the potential to do so), you must report it as soon as possible. This may prevent similar incidents from occurring and aids the Safety Department in the investigation process.
NOTE: If this incident involves violence or harassment, it MUST be reported to the HR department immediately and NOT on this form.
